Using the image shortcode, images using external urls do not appear on the blog post page or static pages (I just see the frame). Mousing over the blank image frame still causes the icon to appear, and clicking on the blank image frame navigates to the image properly.
I turned all plugins off but the problem still occurs. The security plugin makes changes to htaccess and wp-config, so I thought it might still be the source of the problem (can't think of what else it might be).
Home page has image shortcode with remote images, and it works fine.

1) turn off the security plugin. 2) rename the htacces to .htaccess2 3) goto the permalinks in the admin and press save settings a new htaccess will be created.
4) if the images work then you know its the htacces.
if everything stops delete the new htaccess and rename the other one back.
Thanks Paul. The problem still occurs after following the above, so I guess htaccess is not the problem. I also tried reinstalling both the theme and the child theme with no success. So if it isn't plugins, htaccess or the theme files, what can I try next?
a) were the images working correctly before you installed the plugin? and b) were they working correctly after you updated your striking and wp to the latest versions? c) did you update to the latest version of wp while you were running the security plugin? Did you turn off the security plugin when you did the wp update?
I think you have a hard rule written to your database or a db corruption, and what you will have to do is reinstall wp. But you may even have to go to your host, and get a back up to restore your site. The issue could even be a conflict between the plugin and your host server setup. The problem is there are to many possibilities, and you don't want to spend "years" trying to track it down.
But before you go the full backup restore routine. Try this
a) deactivate all plugins b) deactivate the child theme c) delete the .htacess file in your host c) set permalinks to the default and save. e) reinstall the latest version of Striking 5.1.8.3 by ftp f) reinstall the latest version of wp, by ftp g) clear your cache, go to a panel such as the color panel and just save it. h) check your site. Are your images back? g) input the standard /%category%/%postname%/ in the custom permalinks field and save (this will rewrite a new .htaccess file - do you still have your images?
If not, then my gut says you may have to install a from a backup.
Striking 5.1.8.3 and WP 3.4.1 always work. No exceptions. On linux hosting. Once one adds anything else to the mix, other then askimet plugin (which always works with Striking), the user has to do their homework. Now plugins like yoast SEO are also not a problem either, but our certification ends with Striking 5.1.8.3, WP 3.4.1, and a child installed without any changes input yet into the child, and not other changes to one's site.
We will try to provide some additional input as you report back.
I did a completely clean install -- created a new subdomain (test.eimc.ca), installed WP (using Fantastico to install WP 3.3.2 and upgrading to 3.4.1 from within WP) into an empty directory with a new database. Installed striking and striking-child, activated the child theme and inserted an image shortcode on the sample page.
Annnnd.... the problem persists.
I suppose it's possible that images were never working properly, and I just thought they were because the home page images were working.
So does that narrow it down to a host server conflict? According to my cPanel, it is a linux server running Apache 2.2.22, PHP 5.3.10, MySQL 5.1.63cll.
I think you have done all you can. Shoot me over your wp and ftp by private message, along with your skype id, and I will contact you. Its too bad, we are still about 3 weeks out from releasing our own hosting service, with our own servers, specifically set up for wp hosting, and no need for backups, cache plugins, security plugins, and specially configured for ease of use with wp. Any striking user who buys hosting would get it with Striking all setup and no worries about any of this stuff. We have our own private cloud, separate servers only running backups and no oversold servers trying to host umpteem number of sites. We just have to finish getting our cdn in place to our satisfaction, and merchant services in place for accepting payments.

"We can't really offer much support for wordpress shortcode issues, but if there is a specific server setting that is causing the issue, we can change it for you. A lot of issues with using wordpress and plugins has to do with the mod_security and mod_rewrite modules. If you wish us to change or disable either of these, just let us know and we can do that for you."
